IN-NETWORK INSURANCE

Most major insurances are accepted (see below). Your fees are determined by the particular plan you have. My office manager will obtain the necessary information from you when you contact us in order to check your insurance coverage BEFORE your appointment whenever possible.

  • BayCare/EAP
  • BCBS (FL Blue)
  • ChampVA
  • Cigna
  • Medicare
  • Military One Source
  • SpringHealth EAP
  • Tricare Select
  • United Healthcare/UMR
  • Vocational Rehabilitation
  • Wellspring EAP

OUT OF NETWORK & PPO

If your insurance is not listed here and you have a PPO policy, call your insurance company to check your plan’s “Out of Network” benefits. Reimbursify enables individual patients and out-of-network practitioners to file out-of-network claims rapidly and efficiently in their mobile app.

SLIDING FEE SCALE

I offer a sliding scale fee schedule for those who do not have or are unable to use their insurance coverage for therapy.

A sliding scale fee schedule means that you pay what you can afford based on income and household size. If your financial situation changes for the better or worse during our work together, please let me know so we can adjust your rate accordingly. If you wish to use the sliding scale fee schedule, my office manager can help you determine where you fall on the scale when you call for an appointment.

If you can’t afford my sliding scale, check out Open Path Collective.

***PLEASE NOTE*** There is an $85 out of pocket fee (even if you are using insurance) for the following:

  • You cancel your session under 24 hours
  • You are more than 10 minutes late to your session
  • If the insured client does not show up for a couples/family session

GOOD FAITH ESTIMATE

Under the law, health care providers need to give clients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.

You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service, or at any time during treatment.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

For questions or more information about your right to a Good Faith Estimate, or how to dispute a bill, see your Estimate, or visit www.cms.gov/nosurprises.

Getting Started

Choosing a therapist that feels right to you is absolutely essential- it’s the single most important indicator of successful therapy outcomes. Likewise, I will want to make sure that your needs and my expertise are a good fit. My goal is to help you get what you need out of counseling, even if what you need is a referral to another therapist/agency who specializes in a particular area or to someone with whom you may have a stronger connection with. I always welcome any questions you may like to ask or feedback about your experience working together. I offer a free brief telephone consultation if you would like to discuss your interest in therapy in more detail. Finally, I encourage you to review the Forms/Consents, Welcome Packet, and Next Session Worksheet (links below) to get a better feel for my style of therapy and practice policies.

Please note, I do not assure availability at all times and the practice is not geared to the provision of emergency services.

My wonderful office manager, Jennifer, will take care to help you get started, answer billing questions, book appointments, and quickly respond to any other questions you may have along the way. She is available Mondays through Fridays from 9am-4pm.

Forms & Consents

Once an appointment is set, I will need to gather some basic information prior to our meeting. We use a HIPAA-compliant service called Simple Practice for all of our forms and documents. You will access and digitally complete these forms via a secure link sent to you by my Office Manager. It is very important that all paperwork be completed prior to your appointment so that your session time can be used for therapy rather than administrative matters. Copies of the documents are included below for quick reference.
